Description of your identification policy: 
Individuals should be able to provide a valid government-issued document with a photograph, such as a driver’s license issued by the state in which the person resides, state issued ID card, U.S. military ID, U.S. passport, or alien registration card with photograph. If the person is elderly or disabled or does not have a valid government issued document with a photograph, then a primary (e.g. Social Security card, Medicare card, etc.) and secondary document (e.g. utility bill, senior citizens ID, etc.) must be provided for verification purposes
